Nurse Mentorship Program

Home Options Network is offering a nurse mentorship program, focused on developing the appropriate clinical and practical skillset of home nurses.

The mentorship program will consist of managing and training patients, home clinic management, Quality Assurance Performance Improvement (QAPI) and understanding of CMS Conditions for Coverage.

Compliance Audit Review

This is a survey readiness audit to ensure your home program complies with CMS Conditions for Coverage and State regulations.

Reimbursement and Accounts Receivable Audit Review

Review consists of revenue capture, charge master analysis, and A/R collection performance of department/ billing agency.

Adequacy, Prescription and Supply Utilization Review

Review of ordering practices for home supplies, patient adequacies Kt/V, PET and resulting PD prescriptions, with recommendations for maximizing patient outcomes and operational efficiencies.

Survey/Certification Start-Up Services

Services included: assistance with identifying site location, completing State/Federal enrollment applications, interviewing potential employees. Provide comprehensive Home PD Policy and Procedures, clinical and administrative forms, template contracts and job descriptions. Perform readiness mock survey prior to actual licensure/certification survey.

Survey & Compliance Deficiencies/ Resolution & Plan of Correction

Provide home program with guidance and support to address survey deficiencies with a plan of correction to demonstrate compliance with CMS Standards.

Quality Assurance Performance Improvement Program (QAPI) Data

Review of QAPI process for initial or existing home facility programs. This includes: data collection/analysis, trending reports, conducting QAPI meetings, and action plans required to meet CMS Conditions for Coverage.

CKD/Treatment Modality Education

CKD Patient Education Program that trains staff how to effectively conduct CKD patient education. Program includes a customizable treatment option presentation, sample physician and patient letters as well as patient tracking documents.
